Believers compare the profile view from a 12-week (NT) scan against a mental checklist. The "male" markers usually listed are:

  • A defined brow ridge — the bony shelf above the eye sockets, giving a sloped forehead
  • A squarer chin and a more angled jaw
  • A steeper, more vertical forehead line

And the "female" markers:

  • A rounder skull with a smoother, more curved forehead
  • A heart-shaped face, tapering to a narrower, softer chin
  • More prominent cheeks relative to the jaw

Proponents put accuracy anywhere from 65% to 95% depending on who's selling the reading. That spread alone is a signal: methods with real evidence converge on a number, and unverified ones don't.

Where Skull Theory Came From

Those characteristics are driven largely by pubertal hormones; research on cranial sexual dimorphism finds that the distinguishing features either aren't present before puberty or only emerge across childhood growth. Sexing a subadult skull is notoriously unreliable even with the actual bone in hand and calipers on it. Skull theory asks you to do it at 12 weeks, from a grainy profile, by eye.

The honest complication — and it still doesn't rescue the theory. There is a measurable average sex difference in fetal head size: ultrasound biometry studies find male fetuses have slightly larger head circumference and biparietal diameter, on the order of 3% from around 16 weeks, widening later in pregnancy. But an average difference is not a test. The distributions overlap almost completely — plenty of girls have larger heads than plenty of boys — so a 3% shift in a population mean tells you nothing about your individual baby. And it appears after 16 weeks, whereas skull theory is applied at 12, when the whole head measures roughly 80mm around. A three-percent difference on a head that small is a couple of millimetres: far less than the variation introduced by the baby's angle to the probe, the exact dating of the pregnancy, and the machine's rendering settings.

Which brings up the practical problem: the same baby looks different from frame to frame. Tilt the head slightly toward the probe and the forehead reads as steeper. Catch it during a movement and the jaw looks squarer. Change the gain and the brow gains a shadow that reads as a ridge. Parents comparing two scans of their own baby often reach two different skull-theory verdicts — which tells you how much of the "signal" belongs to the imaging rather than to the baby.

The 12-week appointment isn't a dead end for gender clues — it just holds a different one.

Peer-reviewed studies put accuracy around 87–92% at 12 weeks for trained readers on a clean side profile, climbing toward 98–100% by 13 weeks.
